If I breed a 31/31/31/15/11/31 with a Power Weight and a 3/31/31/31/31/31 with a Destiny Knot, what's the chance of me getting perfect IVs?

You should hatch a 5IV parent to replace your 4IV parent before you start thinking about hatching 6IV.

First of all, you should keep trying to get a second parent with 5 IVs, your odds will be MUCH better. Secondly, to actually answer your question, about 0.9766%. I plugged all your IVs into the only IV calculator I could find that accepts both items.
