Leftovers are pretty much always the best item for a tank/wall Pokemon to hold. Sitrus berries are also good, especially if your Pokemon has the harvest ability or recycle. Black sludge is best if you have a poison type as your wall, in place of leftovers. A toxic orb is essential if your Pokemon has poison heal. It mostly depends on the Pokemon and the set you're using.

Another item worth mentioning is rocky helmet. It's mostly useful for chip damage on the switch ins, and goes very well with tanky rough skin Garchomp or iron barbs Ferrothorn.
