Gen IV:

  • Mega-Garchomp
  • Mega-Lucario

Gen V:

None.

Gen VI:

None.

Unconfirmed:

  • Mega-Galvantula (Mega-Stone Found)

Rumoured:

  • Mega-Arcanine (Mega-Stone found, but Arcanine nor Growlithe are found in Kalos.)
  • Mega-Druddigon (Apparently, Mega-Stone found.)

Note that Gen VI Pokémon will not get Mega-Evolution. As for now, Galvantula may be the only Gen V Pokémon able to Mega-Evolve.

Hope this helps.

Serebii says that they got a elite hacker to find out how Diance, Hoopa and Volcanion. They also found a latas and Latios Maega-Evo, but, they look the same to me.
This cannot be.
There is also a rumored Mega Weavile BTW.
